Of every Doctor mandate type currently open on our desk across the GCC, one category has grown faster than any other this year: the concierge or longevity physician appointed directly to a single private family. It is now the single largest recurring theme in our live physician pipeline, ahead of any hospital-based specialty.
These appointments are not a smaller version of hospital medicine. They are a different discipline — preventative, relationship-led, and built around one exclusive, highly restricted patient panel rather than a clinic list.
What is actually being commissioned
Private families and their family offices across Dubai, Riyadh and Jeddah are appointing physicians as directors of a private, bespoke health function — not as on-call cover, but as the architect of a family’s preventative and longevity strategy. The remit typically spans chronic disease optimisation, advanced screening architecture, regenerative and ageing-reversal protocols, and direct leadership of a small private team of physiotherapists, nurses and nutritionists.
The three variants we see most often
Concierge physician to a private family. A live-in 1:1 clinical ecosystem, with the physician directing preventative strategy and travelling internationally with the family. When the family travels, the physician keeps their UK or European registration and coordinates care with local clinics. See our Private Concierge Physician listing.
Sports medicine and longevity specialist. Physical health, regenerative recovery and preventive longevity regimens for a single family, often directing an internal physiotherapy and nursing team. See our Concierge Sports Medicine & Longevity Physician listing.
Private family longevity and family medicine director. The most senior variant, typically inside the family’s private medical office, combining family medicine with hyper-customised longevity protocol design for an exclusive, restricted patient panel. See our Consultant in Family Medicine & Longevity listing.
Why demand has moved this fast
Two forces are compounding. Gulf private families are investing heavily in longevity and preventative medicine as a category in its own right, not as an adjunct to acute care — a global trend that is arriving in the GCC at real scale. At the same time, families who already commission private nursing and physiotherapy teams are consolidating that structure under a single physician director, rather than coordinating specialists independently. The result is a category of appointment that barely existed three years ago and is now among the highest-value mandates we run.
Packages and what they reflect
| Mandate type | Package |
|---|---|
| Concierge physician, Private Family, Dubai | £240,000 tax-free, liquid |
| Sports medicine & longevity specialist, Private Family, Dubai | £240,000 tax-free |
| Family medicine & longevity director, Private Family Estate, Riyadh | £240,000 tax-free, liquid |
Every physician appointment on these mandates is paid £240,000 a year, tax-free, with accommodation and travel included. These packages consistently outpace equivalent hospital-based consultant roles, because continuity, discretion and total clinical authority over the design of the programme are priced directly — not because the clinical work itself is necessarily more complex than tertiary hospital practice.
Who is shortlisted
Tier-1 board certification — CCT/CCST or an equivalent Nordic or European designation — in Family Medicine, Internal Medicine, Sports Medicine or a closely related field, with five to seven years post-certification experience, ideally including prior concierge medicine, executive health or elite screening work. Diplomatic composure and absolute commitment to NDA-governed confidentiality matter as much as the clinical credential.
