The appointment lands wherever the first credible offer arrives, and a royal wing in Abu Dhabi and a family office in Dubai are not interchangeable postings. For a package of £200,000 to £410,000, tax-free and paid net, that is an expensive decision to make by accident — and for a consultant deciding where to spend the next decade, it is worse.
Abu Dhabi differs from Dubai in ways that matter to a senior clinician. The families who retain physicians are weighted more heavily toward Emirati nationals and the ruling houses. The receiving hospitals a household uses are fewer and larger. Sovereign and royal-wing provision is a substantially bigger share of the demand. And the Department of Health licenses independently — a Dubai licence does not carry across.
What Abu Dhabi’s households are actually buying

The receiving hospitals behind Abu Dhabi’s royal wings are built at a scale that supports work a Dubai household physician must often refer out.
Medical leadership. Chief medical consultant and medical director mandates recur here more than anywhere else we recruit, because sovereign clinical commands and royal medical wings are larger than a single household office and the leadership layer is correspondingly deeper.
